1. Team name (you can change it later.)
2. Captain's name, address, phone number
3. Contact's name, e-mail (The team contact must be able to use MS Word and Excell.)
4. The number of players (You must have 4 men and 4 women at least.)

Fee
5,000 Yen for each player (included Players kits which should be GAIA product)
